Develop life care plans at the request of insurance companies and attorneys for the purpose of outlining current and future medical care and associated costs for an individual. Each plan is developed specifically to address the needs of the individual for whom it is requested.

Provide medical/legal consulting and expert testimony in the areas of brain injury, spinal cord injury, burns, multiple trauma, general rehabilitation and life care planning.

Medical case manager responsibilities include coordinating care between a variety of providers, such as physicians, rehabilitation therapists, home health agencies, inpatient and outpatient treatment facilities, and with the family and client to assure quality, holistic, proactive medical and rehabilitative care.
 

Developed and improved clinical aspects of the Transitional Care Units, including medical coverage, rehabilitation therapies, nursing practice, social and support services. Established specialty areas and coordinated a team approach between disciplines. Worked with referral sources to determine appropriate referrals, performed preadmission evaluations, assessed patient care needs and established fees for service.
 

Provided direct patient care for brain injury, stroke, spinal cord injury and multiple trauma patients, and made daily assignments for other RNs, LPNs and CNAs. Participated in a number of committees, including policy and procedure, CARF Accreditation process, Brain Injury, and Career Ladder Revision. Developed and presented Brain Injury Learning Sessions to families, presented the nursing portion of stroke workshops to staff and families, presented the Skin Care segment of the Spinal Injury Learning Series. Worked with rehabilitation therapists to provide holistic patient care. Assisted social workers in conducting Head Injury Support Group. Provided individual teaching to family members and nursing staff.
